Manila is Vaccinating A4 Individuals, Here's How to Register

The city of Manila on Tuesday began vaccinating economic frontliners in the A4 category at different malls as the country's vaccination program expanded to essential workers.

Medical frontliners (A1), senior citizens (A2), and those with comorbidities (A3) are in line to receive the vaccine. Those in the A4 category can sign up online for a schedule online and receive their first dose at Lucky Chinatown Mall, SM Manila, SM San Lazaro, and Robinsons Place Manila.

As of Wednesday, a total of 1,000 doses are alloted to each mall.

Here's how to register for the vaccine in Manila City:

Once you've filled out the form, a QR code will be displayed. Present your printed waiver form or QR code for verification on the day of your appointment.

Those aged 18 to 59 years old with comorbidities must present one of the following prior to being vaccinated:

  • Medical certificate (issued within the last 18 months)
  • Prescribed medicines or doctor's prescriptions (within the last 6 months)
  • Hospital records such as discharge summary or medical abstract
  • Surgical or pathology records
